The Social Determinants of Health

Providers don't work with other community resources to meet the needs of their patients, yet the Center for Medicare and Medicaid Services (CMS) is offering primary care practices who qualify $20 per patient per month for "non-clinical care coordination."

They should buttonhole their administrators to get this money to invest in this critical part of patient care. Why don't they ask for help and collaborate?
